Confidentiality Agreement and Protection of Personal Identifying Information:

I understand that I am providing limited-scope assistance to clinic applicants and am not representing them in their Temporary Protected Status (TPS) application. I understand that I am bound by the duty of confidentiality and cannot divulge any information without the express consent of the applicant.  Information about applicants may not be given to the applicant's family members without the applicant’s permission.  No documents with information pertaining to the applicant's case should be discarded without being properly shredded or otherwise destroyed.  Electronic documents must be deleted following the clinic shift.  I hereby agree to maintain confidentiality in my assignment with the Office of New Americans and AI Justice.
